Report Criteria:

• Active patients (Applied and Permanent)
• Aged 18y or over
• HBA1c in last 12 months with result between 42 (inc) and 47 (inc) or 6.0% and 6.4%
• Fasting Glucose in last 12 months with result between 5.5 (inc) and 6.9 (inc)
• Exclude patients with the following Read codes (ever)
